Madden, Bonnie ~ 92, of Hazard, died Tuesday, December 2 at her residence.
She was the daughter of the late John Lusk and the late Margaret Smith. She was
preceded in death by her sisters Thelma Brown, Freida Fulp and Grada Price and
brothers Creal Lusk and Gurna Lusk. She is survived by a special nephew and his
family, Steve Shepherd and Cathy and children Stephen Carl and Carmen Leigh;
sister Gaytheal Salyers and a host of nieces, nephews, family and friends.
Arrangements were handled by Maggard Brothers Funeral Home of Hazard where the
family received friends on Thursday, December 4. The funeral service was
conducted at the funeral home at 11 a.m. on Friday, December 5 with Rev. Dana

Madden, Homer -born December 27, 1920 in Hazard, departed this life
on Wednesday, November 19 at the University of Louisville Hospital. At the time
of his passing he was a resident of Charlestown, In. and a former resident of
Hazard. He was retired from the U.S. Corps of Engineers and served in the U.S.
Army in WWII. He was also a member of the First Baptist Church in Charlestown.
He is preceded in death by his parents, Charles Guy and Margaret Hardugree
Madden and son William Buddy Madden. He is survived by his loving wife, Mrs.
Edna Combs Madden of Charlestown; sons David E. Madden and wife Jackie of
Charlestown and Roy W. Madden and wife Debbie of Richmond; brothers Ray Madden
of Hamilton and David G. Madden of Hillsborough, Ohio; sisters Etta Roy of
Sapulpa Ok., Ruby Grigsby of Cincinnati and Sadie Hensley of Highland Heights,
Ky.; grandchildren Michelle Madden Cantrell and husband Chet and Scott Madden
and wife Megan and great grandson
Eric Scott Madden. He is also survived by a host of other relatives and
friends.The visitation was held Saturday, November 22 at the Engle-Walker
Funeral Home of Hazard. Funeral services were held at the funeral home with
Glenn Now officiating. The burial took place in Riverside Cemetery in Hazard
with full military honors being performed by the Hazard DAV Chapter No. 64.

Maggard, Elizabeth Wells age 70, of St. Clair Township, OH died at her residence on November 16, 2008. She was born in Hazard, KY on July 21, 1938, the daughter of the late Jerry and Bertha (Spencer) Wells. She received her education and graduated from the Kentucky school systems. On December 29, 1958, in Wooten, KY Elizabeth married Lowell Maggard. She retired in 2006 from Mitsubishi Car Company as a Line Leader. She was a member of the East Hamilton Baptist Church. Elizabeth is survived by her husband of almost 50 years, Lowell; daughter, Peggy Sue Riley and her husband James of Hamilton, OH; four grandchildren Kelly (Scott) Creager, Kimberly Riley, Kristopher Riley and Kevin Riley; great-granddaughter Emily Creager; sister Juanita (Bill) Woods of Indianapolis, IN; brother, Jerry (Trudy) Wells Jr. of Hazard, KY; special sister-in-law Claudia Rust of Hamilton; sisters-in-law Doris Bowling of Cindi, KY, Janice Coots of Hamilton and Eulene Cawsey of Cindi, KY; brother-in-law Ed Maggard of Wooten, KY and many loving nieces, nephews and friends. Maggard, Otis Parnell ~ age 66, of Ary, passed away Friday, March 13, 2009,
at the Hazard Appalachian Regional Medical Center. He was retired from the U.S.
Army Corp of Engineers. He was a member of the Jeff Missionary Baptist Church.

Maggard, Marvin -age 59, of Yerkes, Ky., died Sunday, July 20 in
Yerkes. He was the son of the late Hobert Maggard and the late Bertha Maggard

Matthew, William Henry ~ 65, of Pikeville, died Thursday, December 11 at
the Parkview Nursing Home. Born December 7, 1943 in Hattiesburg, Miss., Mr.
Matthew was the son of the late William Burleigh Matthew and the late Ruth
Sue Adams Matthew. He was a Kentucky Colonel, a member of the American
Legion and had a family practice in Whitesburg. He graduated from the
Louisville School of Medicine and was awarded for accomplishments in the
treatment in Hemophilia in Texas. He was a veteran of the United States Army
and practiced medicine in Tennessee, at the Hazard ARH Medical Center,
Mountain Comprehensive Care, and So.
Williamson Mountain Comp. He was a member of the Pikeville Wesleyan Church.
He is survived by his wife, Clyda Everman Matthew; stepson Jeremy Baker of
Johnson City, Tn.; daughters Suzanne Kayser of Jonesborough, Tn;
stepdaughter Kelly Baker of Knoxville, Tn.; brother Burleigh Matthew of
Scottsdale, Arizona and grandchildren Ian Kayser and Luke Kayser. The
funeral services were conducted on Saturday, December 13 at the J.W. Call
Funeral Home Chapel with Nathan McBeth and Jack Carter officiating. Burial
was at Hill Crest Burial Park in Grayson, Ky. on Sunday, December 14.

McAllister, Kathryn J. -born September 17, 1925, departed this life
on Tuesday, July 29 at her home in Hazard. She was 82 years, 10 months and
12 days of age at the time of her passing. She was born in Knox County, Ky.
the daughter of the late William Avery McAllister and the late Olive
McKinney McAllsiter. She had served the City of Hazard since 1976, and seven
years after her association with city government she was named clerk of the
City of Hazard. Prior to her years in city government, she was on the staff
at Union College as well as youth worker, secretary and director of
religious education at Bowman Memorial United Methodist Church. Kathryn had
also worked for several coal companies. A long time member of Hazard’s
Bowman Memorial United Methodist Church, Ms. McAllsiter played the piano for
the Lotts Creek Evangelical Free Church. She played the piano for Sunday
school since she was 12 years old. She was preceded in death by one brother,
William Avery McAllister, Jr., and by one sister, Jeanette Shepherd. She
leaves the following relatives surviving: nephew Scotty McAllister of
Hazard, nieces Karen Park of Cincinnati, Ohio and Sandy Smith of Cincinnati;
great nieces and nephews Lindsey Philpot, Colton Pitman, Paige Pitman, Gary
Smith II, Kristan Smith and Peggy Steele; first cousins Virginia Stephens of
Hazard and Betty Wright of Lexington; cousins Susan Stephens of Hazard and
the Honorable Judge Leigh Ann Stephens of Hazard, and her beloved cat,
Roxie. She is also survive by a host of other relatives and friends. Funeral
services were conducted on Friday, August 1 at the Bowman Memorial United
Methodist Church with Ray Snider and Mike Cox officiating. The burial
followed in the Resthaven Cemetery in Jeff. The visitation was held at the
church on Thursday, July 31.

McCuistion, Othel Murphy ~ passed peacefully July 19, 2009 surrounded
by her husband, children and family. Mom was born March 14, 1943 in Hazard, KY.
Married for 47 years to our wonderful Father, James L. McCuistion where they
currently reside in Troy, OH for the last 33 years. Her sons, Tony, Derek, Mark

McIntosh, Charles W. ~ born January 2, 1910, departed this life on
Thursday, April 16 at his residence at Sam Campbell Branch Road. He was 99
years, 3 months, and 14 days of age at the time of his passing. He was a life
long resident of Perry County and the son of the late Jerry McIntosh and Sophia
Frost McIntosh. He was a long time member of the Brethren Church. He was a
World War II veteran serving with the United States
Army. Charles was a retired teacher and administrator for the Perry County
School System, having taught for 35 years at the following schools: Lead
Branch, Napfor, Campbell Bend, Meadow Branch, Dunraven, Bonnyman, and Dennis C.
Wooton Elementary. He was principal for Chavies, Big Creek, Combs, and Viper
schools. Charles was also preceded in death by two daughters: Peggy Morgan and
Mattie Couch, three grandchildren: Wesley Whitt, Sherri and Jason Christie;
four great grandchildren: Brittani Gossett, Holden, Cole, and Kendall Christie;
five sisters, and six brothers. He is survived by his wife of 67 years, Sarena
Campbell McIntosh; daughter Linda Whitt of Bonnyman, Kentucky; sister Grace
Reed of London, Kentucky; grandchildren Teresa and Kurt Marten of Wichita
Falls, Texas; Angela and Scott Cross of Knoxville, Tn.; Lesley Couch and Tom
Hoagwood of Nashville, Tn.; Chester Marlowe and Jennifer Akers of Bonnyman;
great-grandchildren Charles, Jessica, Christina Harlow and Caroline Gellner of

McIntosh, David ~ 84, of Snyder, Texas, passed away Tuesday, April 14 at Cogdell
Memorial Hospital in Snyder after a brief illness. He was born on August 6,
1924 in Krypton. He was a Snyder resident since 1956 and served in the U.S.
Navy on board the U.S.S. Edward C. Daly and
U.S.S. Salamaua during WWII. He later served in the U.S. Air Force and retired
from the U.S. Army. Her served as Capt. Snyder with the Snyder Fire Dept. and
operated Snyder Ambulance Service. He was also retired from Chevron. He was
preceded in death by his wife, Otha Sharp, whom he married on May 24, 1966 in
Colorado City. He is survived by his cousin, Nancy Johnson of Hazard. Funeral
services were conducted Saturday, April 18 at Bell Cypert Seale Chapel with Dr.
Tommy Calwell officiating.

Medaris, Ann Moore -age 87, passed away at her home on July 18, 2008. She was born on June 4, 1921. Her parents, Anna May Moore and Tom Moore, preceded her in death. She attended Hazard High School and was valedictorian of her class. She attended college at Transylvania and went to work for the War Department in Washington after graduation. In 1946 she married Frank Medaris and they began their life together in Perry County. She was a full time homemaker and mother until 1965 when she went to work for the State of Kentucky as a social worker. When she retired she was the manager of the Hazard Social Services office. Her husband of 52 years preceded her in death on Dec 26, 1998. During her life Ms. Medaris volunteered a substantial amount of time serving the American Red Cross, the Wilderness Road Council of the Girls Scouts of America, Hazard Perry County Community Ministries, Hazard Perry County Performing Arts Series, and as a tutor at Roy G. Eversole Elementary Schools. She was a troop leader of Brownie and Girl Scout Troops in Hazard for seven years. She was also active with the Transylvania Alumni Association and Chi Omega Sorority of which she was a member while at Transylvania. She is survived by her three children, Margaret Kerns of Louisville, Frank C. ("Rookie") Medaris of Hazard, and Tommy Medaris of Hazard, one son-in-law John Kerns, her daugthers-in- law Carol Medaris and Susie Medaris, five grandchildren Bryan, Tyler, Eric, Travis and Cody, her niece Marilyn MacLeod, and a host of cousins and wonderful friends. She will want to be remembered as someone who was interested in everything and everybody and whose focus was always on others and not herself. In lieu of flowers, donations to Hazard Perry County Community Ministries, the American Red Cross or the Wilderness Council of the Girl Scouts of American are suggested. September 8 at her residence in Krypton. She was 94 years, one month and 23
days of age at the time of her passing. She was born in Krypton, the
daughter of the late Ace Muncy and the late Cassie Pennington Muncy. She
was a life long resident of Perry County and was a member of the Krypton
Brethren Church. She was preceded in death by her husbands, Hubert Campbell
and Burrell Merrill; children Laverne Whitt, Virginia Campbell and James

Moore, Mrs. Essie born July 31, 1922, departed this life on September
22, 2008 at the age of 86 years, one month and 22 days old. She was born at
Dice, Kentucky the daughter of the late Alex Combs and the late Cora Neace
Combs. By occupation she was a homemaker and had been a life-long resident of
Perry County. She was a member of the Bethany Community Church. Some of her

Moore, Junior ~ born September 30, 1923, passed away on Wednesday, March 25 at
the Eastern Kentucky Veterans Center. He was 85 years of age at the time of his
passing. He was born at Hilltown, Ky., the son of the late J.E. Moore and Nelly
Thompkins Moore. He was a World War II veteran and received the American
Defense Ribbon and the Victory Medal. He was preceded in death by his parents
and one sister, May Hurt; brothers Charley Moore, Virgil Moore, Henry Dewey
Moore, Albert Rucil Moore, George Edward Moore, and Johnnie Franklin Moore. He
is survived by half-brothers Fred, Beni, and Manual Moore; half-sisters Agnes

Moore, Lahoma Barnes ~ age 63, of Middletown, OH passed away September 13,
2009 at Atrium Medical Center where she had been a patient for four days. She
was born March 7, 1946 in Hazard, KY and lived in Middletown most of her life.
Preceding her in death were her parents, Algin and Maxine (King) Barnes, her
husband, Fred W. Moore in 1999 and two grandsons, Daehon William Moore and
Christopher J. Harrison. She is survived by two daughters, Arlene Harrison and
husband William P. of Gratis, OH and Kimberly Combs and husband Floyd R. of
Morgan County, KY; one son Fred Moore of Middletown, OH; seven grandchildren,

Moore, Raymond ~ 72, of Lexington, Ky. died Wednesday, December 10 at home with
his family. Ray was born in Darfork, Ky. in Perry County. He was preceded in
death by his parents, Virgil and Eunice Fraley Moore and his sister Nellie Lou
Long. He is survived by his wife, Carolyn Brockman Moore; his sister Majorie
Foster; his brother Virgil Moore Jr.; His son Jonathan Scott Moore and
daughter-in-law Celia Rose Gulick Moore, his future grandson Judah William
Moore, his daughter Jennifer Raye Moore Spiak and son-in-law Daniel Garrett
Spiak and three grandchildren- William Keating Spiak, Daniel Jacob Spiak, and
Catherine
Raye Spiak. Ray was a graduate of Devry University in Chicago Illinois. He
served in the United States Army. He retired from IBM in 1990 after working
there for 28 years as a systems analyst. He built several houses in Lexington
and Somerset. He could do anything with his hands. He was a gifted builder,
woodworker, and jack of all trades. He attended Victory Baptist Church of
Lexington. Funeral Service was at 1 p.m., Sunday, December 14, 2008 at the
Potter Funeral Home. Burial in the South Fork Cemetery, Malone. Visitation
from 6:00 – 9:00 p.m. Saturday, December 13, 2008 at the Potter Funeral Home.
Pallbearers: Charles Holtsclaw, William Long, Phillip Moore, Iskander Judeh,

Morris, Mrs. Mittie Daniel ~ born November 17, 1919, departed this life on
December 9 at the age of 89 years and 22 days. She was born at Owsley County,
the daughter of the late Charles Daniel and the late Roseallie Nolan Daniel. By
occupation Mittie was a retired housekeeper having worked at the hospital for
17 years. She had been a life-long resident of Perry County. Mittie was
preceded in death by her parents: Charles and Roseallie Daniel, her husband:
Joe Morris and by a host of brothers and sisters. She leaves surviving the
following relatives: One sister: Beatrice King of Hazard, and a host of nieces,
nephews, cousins and other relatives and friends. Funeral services for Mrs.
Mittie Daniel Morris will be held at 2 p.m. on Saturday, December 13 at the
Engle-Walker Funeral Home at 301 East Main St., Hazard, Kentucky with Dean
Buchanan officiating. Burial will follow in the Riverside Cemetery at Hazard
with the following serving as pallbearers: Mark Stewart, Joe Stewart, John

Mosley, Estill Ray ~ age 69, of Middletown, OH passed away Thursday, October 15,
2009 at Atrium Medical Center. He was born July 15, 1940 in Hazard, KY to Ray
and Mary (Johnson) Mosley. He was a meat cutter for Billingsly Food Land for 12
years, retiring in 1978 and also worked for Black Clawson as a molder. He is
preceded in death by his parents; sisters Mildred Scaff, Grace Mosley; brothers
Kelly and Alfred Mosley. Estill is survived by his son, Jason Mosley of
Middletown, OH; daughters Sarah Louise Toalston and Teresa Lynn Mosley both of
Hanoverton, OH; grandchildren, Donald Toalston, Jr., Sean Michael Golding;
brothers Ernest Mosley of Middletown and Clay Mosley of London, KY; sister Dora
Stacy of Versallies, KY. Visitation will be held October 23, 2009 from 1:00-2:00
PM at Baker-Stevens Funeral Home. A Celebration of Life Service will follow at
2:00 PM at the funeral home with Jerry Minor officiating. Interment will be at

Mosley, Willie Paul ~ born June 25, 1925, departed this life on Tuesday, April
14 at the Hazard ARH medical center. He was 83 years, 9 months, and 20 days of
age at his passing. He was born in Leslie County, the son of the late Lawrence
Mosley and Minnie Smith Mosley. He was a resident of Combs, Kentucky, a coal
miner by occupation, and a member of the Baptist Church. He was preceded in
death by his parents; daughter Joy Sue Whitehead, and son Johnny Mosley. He is
survived by his wife, Rachel Napier Mosley of Combs; sons Alex Mosley and wife
Joyce, Donnie Mosley, and Bill Mosley, all of Combs, Kentucky; daughters Vernie
Holland and husband Chester Ray of Combs, Kentucky and Mary Williams and
husband Jimmy of Sixteen Mile Creek, Kentucky; brother Donnie Mosley of Big
Creek, Kentucky; sisters Ruby Herald and husband Irvin of
Combs, Kentucky and Essie White of London, Kentucky; 6 grandchildren 4 great
grandchildren, and a host of other relatives and friends. Funeral services were
held at the Engle-Walker Funeral Home on East Main Street in Hazard, Kentucky
on Friday. April 17 with Rev. Rick Bolton
officiating. Burial followed in the Mosley family cemetery located at Combs.

Mullins, Albert W.
-age 63, of Carlisle, OH passed away on
Nov. 25, 2008 at his
residence, at the age of 63. He was born in Hazard, KY on
October 28, 1945.
Mr. Mullins worked for the
City Car Terminal in Detroit, MI.
He loved fishing. Survivors include his wife, Sheila; two daughters, Christina
Kile and son-in-law Troy of MI and Melissa Doan and son-in-law Edward of OH; 4
grandchildren, Matthew, Tori, Chelsea, and Haley; 3 stepgrandchildren, Shawn
Doan, Shannon Doan, and Brandon Doan; 2 sisters, Jean and Josephine; and 3
brothers, Randall, Wayne and Billy. He was preceded in death by his mother, Ora;
his father, Jesse; his sister, Maxine; and his brother, Rhondall. Funeral
services will be held at 1:00 PM November 29, at the Breitenbach-McCoy-Leffler
Funeral Home, officiated by the Rev. Robert Fisher. Friends may call from
12 noon until service time. Burial will follow in
Butler County Memorial Park.

Mullins,
Garfield -the husband of Katie Combs Mullins, passed away on Wednesday,
January 30, at the Hazard A.R.H. Hospital. At the time of his death, he was 66
years of age. The son of the late Steve and Hazel Blanton Mullins, he was also
preceded in death by four brothers, Tommy, Boyd, Gatheryn and Jay. Along with
his widow, he is also survived by his daughter, Sherri and husband Tim McCoy of
Tennessee, and his son, Lynn and wife Allison Mullins of Hazard. His surviving
siblings are Faye Baker, Juanita Osborne, Clyde Mullins, Marvin Mullins and
Kermit Mullins. He is also survived by his seven grandchildren, James D.
Feltner, Jessica L. Feltner, Megan J. Mullins, Joshua E. Feltner, Dennis Landry
Mullins, Balea M. McCoy and Timothy Dallas McCoy, J.T. McCoy II, Seth McCoy,
Ryan and Chris Hurt. The family received friends from 11:00 AM until 1:00 PM
Saturday, February 2, at Maggard Brothers Funeral Home where the funeral service
was conducted on Saturday afternoon with Rev. Ellen Peach officiating. Interment
was at the Gayhart Cemetery in Lotts Creek. All arrangements were entrusted to
